Community Health Services (CHS) offers low cost, high quality health care for young children and teenagers. We provide pediatric care year round at our main clinic and in several school-based health centers within Adams 14 during the school year. Services are provided by Pediatric Nurse Practitioners (PNP’s) with a pediatric doctor on staff for consultation.

Community Health Services also offers:

• Keep Me Safe program - car seat checks to check your child's car seat for correct installation. Car seat checks are available by appointment by calling the main clinic at 303.289.1086.
• Community Outreach Program - a program that assists you in filling out Medicaid and CHP+ applications. This program helps you through the process of applying for Medicaid and/or CHP+ for your children.
• A Walk-in Immunization Clinic - a service provided every Wednesday from 2-4 p.m. for children over four years old with no health insurance. Cost is $10 each immunization, due at the time of service. Your child’s immunization record is required.

• CHP+ (Colorado Access)
• Medicaid (Regular and Colorado Access)
• Some Private Health Insurance
• Cash/Check
• Sliding Fee Available with Approved Financial Screening
